Transition services are a coordinated set of activities for a student, designed within an outcome-oriented process, which promote movement from school to post-school activities, including post-secondary education, vocational training, integrated employment (i.e. supported employment), continuing education, adult services, independent living, or community participation. These activities are derived from the individual student’s needs, taking into account the student’s preference and interests. The process begins at sixteen years (or younger) and includes the student, family, education personnel and vocational and adult service providers.
